We are looking for a Case Manager II with a passion for helping young fathers and seeing clients succeed! The Case Manager II will have expertise in working with young fathers around knowledge about effective parenting skills, workforce development, educational resources, mental and physical health, housing and other needs.

We're looking for a Case Manager II who will:
- Bring a positive energy and attitude toward the engagement of both clients and other staff
- Have the ability to balance client services and grant/administrative requirements. Commit to the pursuit of excellence in both.
- Demonstrate patience with clients who struggle to achieve goals
- Have the ability to maintain required documentation and other program data.

The Case Manager II will be balancing a caseload with several clients in crisis among those not in crisis; balancing direct services with paperwork requirements; utilizing community resources.
•Provides case management services on all cases and coordinates services with other agencies. Duties may include intake, case management, monitoring, resource referral, and group facilitation.
•May conduct education/prevention presentations and facilitate support groups.
•Maintains direct contact with clients through home, school, and/or field visits, providing education and assistance in the area of specialty.
•Provides referrals and assists clients in accessing assistance from community resources.
•Communicates with community agencies regarding services and appointments. Transports clients to agencies and services, as needed.
•Collaborates with community agencies to advocate for client population needs.
•Maintains case records and documentation as appropriate, including service plans, progress notes, intake reports, and incident reports.
Qualifications:REQUIRES a Bachelor’s degree in behavioral science or a related field; 3+ years of related experience; or equivalent combination of education and experience.

Prefer at least two years of experience working with fathers or young males. Bilingual in English and Spanish
