ACTS is a case management program designed to assist youth transitioning out of CPS foster care to achieve independent self-sufficiency in the adult community. The services are provided by LifeWorks and Casey Family Programs under contract with DFPS Preperation for Adult Living Program.
We are looking for a Case Manager I with a passion for the client population and seeing clients succeed to provide case management services and training!
The ideal candidate has demonstrated skill in client services, community resource utilization, and working with multi-cultural populations. They can pay careful attention to detail, works w/ accuracy and maintains neat, well-organized records; works independently, exercises initiative and has high administrative skills.
The Case Manager I will provide assistance to clients in a classroom setting, delivers training and provide classroom management for LifeSkills classes, handle intake calls and walkins and provide administrative support
The ideal candidate is someone who is knowledgeable/experienced in the foster care system, working with the homeless population as well as expertise in mental health issues; disability issues, resources, systems.
Schedule: includes regular business hours, evenings and Saturdays (not exceeding 40 hours)
|Duties:||•Provides case management services and coordinates services with other agencies. Duties may include intake, case management, monitoring, group facilitation, and resource referral.|
•May conduct education/prevention presentations and facilitate support groups.
•Maintains direct contact with clients through home, school, and/or field visits, providing education and assistance in the area of specialty.
•Provides referrals and assists clients in accessing assistance from community resources.
•Communicates with community agencies regarding services and appointments. Transports clients to agencies and services, as needed.
•Collaborates with community agencies to advocate for client population needs.
•Maintains case records and documentation as appropriate, including service plans, progress notes, intake reports, incident reports.
a Bachelor’s degree in behavioral sciences or administrative related field; Requires 1+ years of related experience; or equivalent combination of education and experience. Microsoft Office proficiency, 2+ years full time paid experience working with youth
Must possess a valid Texas driver's license and insurance and have a driving record that would permit being insured by agency insurance; Must have or be able to obtain within 30 days of hire standard First-Aid & CPR certification; must be able to pass criminal and DFPS background checks.
Experience with foster care system, administrative experience, case management experience, working with youth/young adults
